India coronavirus: Why is India reopening amid a spike in cases?

India is roaring – rather than inching – back to life amid a record spike in Covid-19 infections. The BBC’s Aparna Alluri finds out why.

On Saturday, India’s government announced plans to end a national lockdown that began on 25 March.

This was expected – the roads, and even the skies, have been busy for the last 10 days since restrictions started to ease for the first time in two months. Many businesses and workplaces are already open, construction has re-started, markets are crowded and parks are filling up. Soon, hotels, restaurants, malls, places of worship, schools and colleges will also reopen.

But the pandemic continues to rage. When India went into lockdown, it had reported 519 confirmed cases and 10 deaths. Now, its case tally has crossed 173,000, with 4,971 deaths. It added nearly 8,000 new cases on Saturday alone – the latest in a slew of record single-day spikes.

So, why the rush to reopen?

The lockdown is simply unaffordable

“It’s certainly time to lift the lockdown,” says Gautam Menon, a professor and researcher on models of infectious diseases.

“Beyond a point, it’s hard to sustain a lockdown that has gone on for so long – economically, socially and psychologically.”

From day one, India’s lockdown came at a huge cost, especially since so many of its people live on a daily wage or close to it. It put food supply chains at risk, cost millions their livelihood, and throttled every kind of business – from car manufacturers to high-end fashion to the corner shop selling tobacco. As the economy sputtered and unemployment rose, India’s growth forecast tumbled to a 30-year-low.

Raghuram Rajan, an economist and former central bank governor, said at the end of April that the country needed to open up quickly, and any further lockdowns would be “devastating”.

The opinion is shared by global consultant Mckinsey, whose report from earlier this month said India’s economy must be “managed alongside persistent infection risks”.

“The original purpose of the lockdowns was to delay the spike so we can put health services and systems in place, so we are able handle the spike [when it comes],” says Dr N Devadasan, a public health expert. “That objective, to a large extent, has been met.”

In the last two months, India has turned stadia, schools and even train coaches into quarantine centres, added and expanded Covid-19 wards in hospitals, and ramped up testing as well as production of protective gear. While grave challenges remain and shortages persist, the consensus seems to be that the government has bought as much time as possible.

“We have used the lockdown period to prepare ourselves… Now is the time to revive the economy,” Delhi Chief Minister Arvind Kejriwal said last week.

The silver lining

For weeks, India’s relatively low Covid-19 numbers baffled experts everywhere. Despite the dense population, disease burden and underfunded public hospitals, there was no deluge of infections or fatalities. Low testing rates explain the former, but not the latter.

In fact, India made global headlines not for its caseload but for its botched handling of the lockdown – millions of informal workers, largely migrants, were left jobless overnight. Scared and unsure, many tried to return home, often desperate enough to walk, cycle or hitchhike across hundreds of kilometres.

Perhaps the choice – between a virus that didn’t appear to be wreaking havoc yet, and a lockdown that certainly was – seemed obvious to the government.

But that is changing quickly as cases shoot up. “I suspect we will keep finding more and more cases, but they will mostly be asymptomatic or will have mild symptoms,” Dr Devadasan says.

The hope – which is also encouraging the government to reopen – is that most of India’s undetected infections are not severe enough to require hospitalisation. And so far, except in Mumbai city, there has been no dearth of hospital beds.

India’s Covid-19 data is spotty and sparse, but what it does have suggests that it hasn’t been as badly hit by the virus as some other countries.

The government, for instance, has been touting India’s mortality rate as a silver lining – at nearly 3%, it’s among the lowest in the world.

But some are unconvinced by that. Dr Jacob John, a prominent virologist, says India has never had, and still doesn’t have, a robust system for recording deaths – in his view, the government is certainly missing Covid-19 deaths because they have no way of knowing of every fatality.

And, he says, “what we must aim for is flattening the mortality curve, not necessarily the epidemic curve”.

Dr John, like several other experts, also predicts a peak in July or August, and believes the country is reopening so quickly because the “government realised the futility of such leaky lockdowns”.

A shift in strategy

So is the government gearing up for another lockdown when the peak comes?

While Dr Menon believes the lockdown was well-timed, he says it was too focused on cases coming from abroad.

“There was a hope that by controlling that, we could prevent epidemic spread, but how effective was our screening [at airports]?”

Now, he adds, is the time for “localised lockdowns”.

The federal government has left it to states to decide where, how and to what extent to lift the lockdown as the virus’ progression varies wildly across India.

Maharashtra alone accounts for more than a third of India’s active cases. Add Tamil Nadu, Gujarat and Delhi, and that makes up 67% of the national total.

But other states – such as Bihar – are already seeing a sharp uptick as migrant workers return home.

“Initially, most of your cases were in the cities,” Dr Devadasan says. “But we kept the migrant workers in cities and didn’t allow them to go home. Now, we are sending them back. We have facilitated transporting the virus from urban areas to rural areas.”

While the government has said how many infections have been avoided – up to 300,000 – and lives saved – up to 71,000 – by the lockdown, there is no indication of what lies ahead.

There is only advice: The day the government began to ease restrictions, Mr Kejriwal tweeted, urging people to “follow discipline and control the coronavirus disease” as it was their “responsibility”.

Because the alternative – of curfews and constant policing – is unsustainable.

“My worry is more the circumstances of people – it’s not as though they have an option to practise social distancing,” Dr Menon says.

And they don’t – not in joint family homes or one-room hovels packed together in slums, not in crowded markets or busy streets where jostling is second nature, or in temples, mosques, weddings or religious processions where more is always merrier.

The overwhelming message is that the virus is here to stay, and we have to learn to live with it – and the only way to do that, it appears, is to let people live with it.

China virus toll hits 41; Australia reports first four cases

BEIJING (Reuters) – The death toll from China’s coronavirus outbreak jumped on Saturday to 41 as the Lunar New Year got off to a gloomy start, with Hong Kong declaring a virus emergency, scrapping celebrations, and restricting links to mainland China.

Australia on Saturday confirmed its first four cases, Malaysia confirmed three and France reported Europe’s first cases on Friday, as health authorities around the world scrambled to prevent a pandemic.

Hong Kong leader Carrie Lam on Saturday declared a virus emergency in the Asian financial hub, with five confirmed cases, immediately halting official visits to mainland China and scrapping official Lunar New Year celebrations.

Inbound and outbound flights and high speed rail trips between Hong Kong and Wuhan, the epicentre of the outbreak, would be halted, and schools, now on Lunar New Year holidays, would remain shut until Feb. 17. The territory was also treating 122 people suspected of having the disease.

The death toll in China rose to 41 on Saturday from 26 a day earlier and more than 1,300 people have been infected globally with a virus traced to a seafood market in the central city of Wuhan that was illegally selling wildlife.

Hu Yinghai, deputy director-general of the Civil Affairs Department in Hubei province, where Wuhan is located, made an appeal on Saturday for masks and protective suits. Hospitals in the city have made similar pleas.

“We are steadily pushing forward the disease control and prevention … But right now we are facing an extremely severe public health crisis,” he told a news briefing.

Vehicles carrying emergency supplies and medical staff for Wuhan would be exempted from tolls and given traffic priority, China’s transportation ministry said on Saturday.

Wuhan said it would ban non-essential vehicles from its downtown starting Sunday to control the spread of the virus, further paralysing a city of 11 million that has been on virtual lockdown since Thursday, with nearly all flights cancelled and checkpoints blocking the main roads leading out of town.

Authorities have since imposed transport restrictions on nearly all of Hubei province, which has a population of 59 million.

In Australia, three men, aged 53, 43 and 35 in New South Wales were in stable condition after they were confirmed to have the virus after returning from Wuhan earlier this month.

A Chinese national in his 50s, who had been in Wuhan, was also in stable condition in a Melbourne hospital after arriving from China on Jan. 19, Victoria Health officials said.

State-run China Global Television Network reported in a tweet on Saturday that a doctor who had been treating patients in Wuhan, 62-year-old Liang Wudong, had died from the virus.

It was not immediately clear if his death was already counted in the official toll of 41, of which 39 were in the central province of Hubei, where Wuhan is located.

U.S. coffee chain Starbucks said on Saturday that it was closing all its outlets in Hubei province for the week-long Lunar New Year holiday, following a similar move by McDonald’s in five Hubei cities.

PROTECTIVE SUITS

In Beijing on Saturday, workers in white protective suits checked temperatures of passengers entering the subway at the central railway station, while some train services in eastern China’s Yangtze River Delta region were suspended, the local railway operator said.

The number of confirmed cases in China stands at 1,287. The virus has also been detected in Thailand, Vietnam, Singapore, Japan, South Korea, Taiwan, Nepal, and the United States.

The U.S. Centers for Disease Control and Prevention said on Friday it had 63 patients under investigation, with two confirmed cases. While China has called for transparency in managing the crisis, after cover-up of the 2002/2003 Severe Acute Respiratory Syndrome spread, officials in Wuhan have come in for criticism over their handling of the current outbreak.

In rare public dissent, a senior journalist at a Hubei provincial newspaper run by the ruling Communist Party on Friday called for a “immediate” change of leadership in Wuhan on the Twitter-like Weibo. The post was later removed.

REINFORCEMENTS TO WUHAN

The World Health Organization (WHO) declared the new coronavirus an “emergency in China” this week but stopped short of declaring it of international concern.

Human-to-human transmission has been observed in the virus.

China’s National Health Commission said it had formed six medical teams totalling 1,230 medical staff to help Wuhan.

Hubei province, where authorities are rushing to build a 1,000 bed hospital in six days to treat patients, announced on Saturday that there were 658 patients affected by the virus in treatment, 57 of whom were critically ill.

The newly-identified coronavirus has created alarm because there are still many unknowns surrounding it, such as how dangerous it is and how easily it spreads between people. It can cause pneumonia, which has been deadly in some cases.

Symptoms include fever, difficulty breathing and coughing. Most of the fatalities have been in elderly patients, many with pre-existing conditions, the WHO said.

NEW YEAR DISRUPTIONS

Airports around the world have stepped up screening of passengers from China, though some health officials and experts have questioned the effectiveness of such screenings.

There are fears the transmission could accelerate as hundreds of millions of Chinese travel during the week-long Lunar New Year holiday, which began on Saturday, although many have cancelled their plans, with airlines and railways in China providing free refunds.

The virus outbreak and efforts to contain it have put a dampener on what is ordinarily a festive time of year.

Sanya, a popular resort destination on the southern Chinese island of Hainan, announced that it was shutting all tourist sites, while the island’s capital city, Haikou, said visitors from Wuhan would be placed under 14-day quarantine in a hotel.

Shanghai Disneyland was closed from Saturday. The theme park has a 100,000 daily capacity and sold out during last year’s Lunar New Year holiday.

Beijing’s Lama Temple, where people traditionally make offerings for the new year, has also closed, as have some other temples.

Fast food chain McDonald’s apologised to a customer who was served milk tea contaminated with disinfectant in one of its stores in eastern China, damaging her digestive tract.
The restaurant said on Monday that it had reached a settlement with the customer, a woman surnamed Huang, after she suffered vomiting, a sore throat, a stomach ache and was taken to hospital on Friday for treatment.
Huang – who was still in hospital – suffered the symptoms shortly after taking a sip of the drink in a restaurant at Changle Airport in Fuzhou, the capital of Fujian province, the Strait Metropolis Daily reported.
“She just had one sip and found it tasted bad. But she already swallowed some of it,” Huang’s husband, surnamed Tang, told the newspaper.
McDonald’s apologises after advertisement showing Taiwan as a country draws criticism

When the lid was removed from the cup there was a strong smell of disinfectant, Tang said.

“I asked staff workers about the drink, but they just took it and smelled it, without giving any explanation. They threw the drink into the rubbish when we were not looking, but we took it back,” Tang said.

Doctors said the woman’s symptoms were caused by sodium dichloroisocyanurate, a chlorine compound widely used to kill bacteria in water by the catering industry and by the leisure sector in swimming pools.

On its Weibo feed, McDonald’s described the incident as unintentional and said staff would be given more training.

“McDonald’s has paid great attention to the incident and is deeply sorry for the error made by a staff member of our restaurant,” it said.
